Robotic thyroidectomy via the bilateral axillo-breast approach (BABA) is a safe, effective alternative to conventional surgery, offering superior cosmesis by avoiding a neck scar. This video showcases our early Indian experience using the indigenously developed SSI Mantra system, detailing space creation, port placement, surgical steps and postoperative outcomes. Twenty-three patients (mean age 32 years; 20 females, 3 males) underwent surgery with a mean nodule size of 2.9 cm and BMI 25.6. Procedures included lobectomy (4), total thyroidectomy (16) and total thyroidectomy with central neck dissection (3). Mean operative and console times were 189 and 121 minutes respectively. Postoperative morbidity was minimal, with transient vocal cord palsy in 4 and transient hypoparathyroidism in 5 patients, all resolving during follow-up. No permanent complications occurred. This experience demonstrates that robotic thyroidectomy using the SSI Mantra robot is feasible, safe and effective, achieving excellent cosmetic and clinical outcomes.
