Urinary incontinence remains a significant quality-of-life concern following radical prostatectomy. In this video, we aim to describe the combination of single-port extraperitoneal robotic radical prostatectomy (SP-EP-RARP) with anatomical hood reconstruction for early continence recovery. The use of the extraperitoneal approach facilitated by the single port robot may not only promote reduced morbidity, less pain, improved cosmesis but also reduce risk of bowel injury by avoiding the peritoneal cavity, improve application in a hostile abdomen, address the limitation of Trendelenburg positioning requirement in patients with medical comorbidities such COPD or obesity and also facilitate higher rates of same-day discharge with minimal narcotic use post-operatively.
