Young women diagnosed with cervical carcinoma, preservation of fertility is a key concern. Radical trachelectomy has emerged as an alternative to radical hysterectomy in carefully selected patients. The oncological outcomes are similar to radical hysterectomy and the pregnancy rates are reported to be 35-45% after radical trachelectomy. Uterine artery ligation can affect reproductive outcomes,hence whenever possible ascending branch of uterine artery should be preserved in radical trachelectomy . This video demonstrates standardised approach to uterine artery preserving radical trachelectomy without compromising parametrial clearance. Parametrial dissection can be difficult after preservation of the uterine artery as you have to work around the vessel to dissect the ureter and resect adequate paarmetrium .Robotic platform with the dexterity not only allows a precise dissection of uterine artery but also allows effective suturing for the uterovaginal anastomosis.
