Traditional radical cystectomy in females entails en bloc removal of the bladder, uterus, adnexae, anterior vaginal wall, and urethra. As surgical focus has shifted towards functional preservation, recent techniques emphasize selective resection, given the low rates (2.5–7.5%) of gynecological organ involvement by bladder pathology. Aim of this presentation is to demonstrate a pelvic organ-sparing radical cystectomy (POSRC) technique in women, with preservation of the uterus, adnexae, anterior vaginal wall, and periurethral support, aiming to optimize postoperative sexual and urinary function. This technique optimizes anatomical and functional outcomes, supporting current trends in minimally invasive and function-preserving surgery.
