Spindle cell lipoma of oesophagus is a rare entity. In our case, the size and location of the tumour was particularly challenging. The site of lesion was at upper and mid-thoracic oesophagus with dimensions of 11x4x4cm. The challenge was about the approach of surgery. Cervical excision was frought with possibility of high and long oesophagotomy to excise a massive lesion like this. Also, the technical challenge of closing a high cervical oesophagetomy limits the feasibility of this procedure, with high incidence of leak rates and fistula formation in a high cervical anastomosis. The robotic transthoracic approach, as performed in our case, was an innovative approach for a rare yet challenging case, at the same time offering the advantages of minimally invasive approach.
