#177 Totally Robotic Revision Bariatric Surgery Dr. Arun Prasad

ABSTRACT Title : Totally Robotic Revision Bariatric Surgery

Introduction : Traditionally laparoscopic assistance has been used to help with the infra colic part of the surgery and also the use of laparoscopic staplers by assistant surgeon to do the resection and anastomosis. We show how the entire procedure can be performed by the console surgeon.

Material and Methods : Revision Bariatric Surgery patients – 20

Use of Intuitive daVinci Xi platform

Use of Sureform stapler

Analysis of operative time taken and 30 day complications seen after surgery. Compared with historical data from our experience.

Results : This video shows the use of DaVinci Xi with Sureform stapler being used for a complex revision bariatric surgery. Traditionally laparoscopic assistance has been used to help with the infra colic part of the surgery and also the use of laparoscopic staplers by assistant surgeon to do the resection and anastomosis. We show how the entire procedure can be performed by the console surgeon. The sureform stapler provides a level 1 AI assistance to decide the stapler height and color by gauging the thickness. Also during the stapling process it uses this knowledge to compress the tissue for desired amount of time before stapling and cutting. This leads to a no bleeding situation from the staple line as is evident from the video. Full control of the procedure has brought our operating time down to 40 minutes in an uncomplicated case. Earlier avareage time of ours with robotic assisted was 50 minutes and with laparoscopy 65 minutes.

Conclusion : Revision Bariatric Bypass Surgery can be performed by this totally robotic technique to improve outcomes and reduce operative time.
