584 A telesurgery session for Robot Assisted Total Hysterectomy

Dr. Pietro Pasquini

Despite recent advances, telesurgery still lacks strong evidence and large clinical series, particularly in gynecology. Published experiences have mostly reported isolated cases, while no complete telesurgical sessions have been documented. Broader validation requires greater resource availability, robust infrastructures, and demonstration of reproducible workflows. In this video, we report for the first time a full telesurgical session in gynecology, consisting of three consecutive Robot-Assisted Total Hysterectomies (RATH) performed remotely. Different surgeons operated through a novel robotic platform, while synchronized technical and surgical setups were recorded, including network delay graphs and operative timings. The innovation lies in showing that telesurgery can be safely conducted for an entire surgical session, without conflicts, technical failures, or complications, and without prolonging operative or turnover times. This experience provides a novel perspective toward establishing feasibility benchmarks and informing future multicenter validation.
