In prostate cancer surgery, especially with suspected extracapsular extension (ECE), it’s difficult to balance complete tumor removal with nerve preservation, risking either recurrence or functional impairment. This study combined 3D augmented reality (AR) and real-time digital pathology (VivaScope) during nerve-sparing robot-assisted radical prostatectomy. AR-guided biopsies identified tumor involvement intraoperatively, and real-time analysis guided precise decisions on whether to preserve or remove nerve bundles. The integration of AI-driven AR imaging with confocal microscopy allowed real-time surgical adjustments, improving oncological outcomes while preserving healthy tissue—marking a shift from static planning to dynamic, precision-guided prostate surgery.
