In advanced rectal cancers, as part of multi modality treatment, radical surgeries such as total pelvic exenteration has been the only therapeutic option. With accurte imaging and advancement in operative techniques and equipment, less radical surgeries are possible now. We present a case of carcinoma rectum with prostatic invasion where we resected the rectum with the prostate as part of exenteration but were able to spare the bladder.
