649 Endoscopic assisted Robotic excision of ovarian cystic teratoma complicated with rectalfistula

A patient presents with a large rectal polyp. It is pedunculated but pedicle is too thick for endoscopic excision. The attachment of the polyp to right ovary not clearly demonstrated and hence complete diagnosis is not clear on Computed Tomography imaging. Operation starts with Diagnostic Laparoscopy. After confirmation of a right ovarian cyst firmly attached to the right wall of rectum, it is decided to procced further with Robotic Surgery. Robot docked and surgery performed with endoscopic guidance. Finally natural orifice specimen extraction done. A complex pelvic surgery with an extremely rare occurence is performed to perfection with the help of Robot assistance and reported for the first time in literature.
