638 Robotics Redefining Reality : Transforming Complex Spinal Deformities

Neuromuscular scoliosis often presents with severe deformity, pelvic obliquity, and poor bone quality, making long-segment fusion and stable spinopelvic fixation challenging. Conventional iliac screws, though widely used, are associated with implant prominence and soft-tissue complications. Solution: S2-alar-iliac (S2AI) screws offer a biomechanically strong, low-profile alternative for spinopelvic fixation. In a 14-year-old girl with progressive neuromuscular scoliosis, robotic-assisted navigation enabled precise preoperative planning and accurate intraoperative screw placement. This ensured ideal trajectories, minimized fluoroscopy use, and achieved stable correction of pelvic obliquity and coronal imbalance. Innovation: The integration of robotic technology with S2AI screw fixation represents the innovation. Robotics enhances precision, reproducibility, and safety in complex trajectories while reducing complications and radiation exposure. This approach not only improves deformity correction but also avoids implant-related issues, offering a transformative solution in complex neuromuscular scoliosis surgery.
